Standard #: 22VAC40-185-270-B Description: Based on observation of the physical plant, heaters were found that did not have appropriate barriers to prevent children from being burned, shocked, or injured from heating equipment. Evidence: In the Prek classroom(4's), 2 portable heater's were found in the classroom turned on and accessible to children. The heaters did not have a barrier around it to prevent children from touching the heater. Plan of Correction: Assistant Director unplugged the heater during this inspection and will move the heater's so that they are inaccessible to children when they are plugged up and turned on.
Standard #: 22VAC40-185-280-H Description: Based on observation of the physical plant, purses were found accessible to children. Evidence: In the 2's classrooms, a purses were found in a cabinet with a broken safety latch. Plan of Correction: Assistant Director stated that she will replace cabinet safety latch and have the purse removed until the cabinet is fixed.
Standard #: 22VAC40-185-340-F Description: Based on observation and interview with Staff, staff failed to provide sight and sound supervision while children were playing on a play structure on the playground. Evidence: A self-reported visit was initiated as a result of a incident that occurred on 3/9/17 while children were playing on the playground. A group of 4 toddler's (4 year old) were on a play structure (play castle)in the far back of the playground, were able to play ''a game"" involving pulling down their pants and showing their private parts. This occured for approximately "one minute" before staff intervened. Center staff failed to provide adequate supervision by circulating and interacting with the children to prevent this from occuring. Plan of Correction: Director stated that they will continue to monitor playground and all the children. Students weretalked to about the importance of keeping private parts, private. Staff will circulate the playground and interact with the children and a staff member will be position near this play structure in the event that it is not removed from the playground.
Standard #: 22VAC40-185-500-B Description: Based on observation of the physical plant, surface for diaper changing was found nonabsorbent. Evidence: in two of the classrooms, the diaper changing pad was found ripped/torn and no longer nonabsorbent. Plan of Correction: Assistant Director stated that the diaper changing pads will be replaced immediately.
