Comments:
An unannounced monitoring inspection was conducted today. There were 55 children in care, with a total of 9 staff members assigned to their direct supervision. The children were observed engaging in a wide variety of creative play activities, making various spring flowers and flower pot artwork, playing with magnet toys, and making seed to tree story strips. This inspection was completed between approximately 10:15 am and 12:00 pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that one record is not complete. Evidence: Child record #3 does not include the home phone number.

Plan of Correction: I will ask the parent for the information.

Standard #: 22VAC40-185-510-C
Description: Based on a review of children's medications, it was determined that two medications were on site without all required information. Evidence: 1. One child's long-term emergency prescription medication expired on 2/28/17. 2. The name of the non-prescription medication on site for one child does not match the name of the non-prescription medication that is listed on the child's authorization form.

Plan of Correction: The parents will be asked for two new medications.
