Comments:
An unannounced monitoring inspection was completed today 3/28/2017. There were 31 children in care on the day of the inspection with seven staff. There were five children's and eight staff's records reviewed during the inspection. A physical plant walk through was completed, and observations of the children. Discussion: hand washing requirements for infants, preschool classroom and timely background checks.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of eight staff's records, the center failed to ensure that all staff had repeat TB screenings every two years. Evidence: 1. The inspector observed that staff # 2 most recent TB screening was 3/6/2015.

Plan of Correction: The administrator will send the staff person to obtain a new TB screening ASAP. All records will be monitored.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure that all area and equipment of the center, inside and outside maintained in a clean, safe and operable condition. Evidence: 1. The inspector observed the laundry basket in the preschool side that was cracked in several places and sharp pieces and needs to be discarded or moved out of the child care area. 2. In the toddler room there was a switch plate that was missing a portion and needs to be replaced. 3. In the preschool room there was switch plate that was cracked and missing a portion and needs to be replaced.

Plan of Correction: The basket was removed during the inspection. The switch plates will be replaced as soon as possible. All areas and equipment will be monitored.
