Comments:
Today's visit was a renewal inspection. This inspection included the subsidy health and safety inspection. The inspection began at 9:30am and concluded at 2:20pm. Upon arrival at the center there were twenty-five children in care with five staff members. During the inspection activities in the multipurpose room were observed, diapering was observed, all three classrooms were viewed, a sample of three out of twelve staff records were reviewed, a sample of three out of seventy-five children's records were reviewed, the outdoor area was viewed, nap time was observed, some policies and procedures were reviewed to include the new lock down procedure, and emergency supplies were viewed for all three levels of the building. Your renewal application has been received in the licensing office. A recommendation regarding your license will be made upon completion of today's inspection. Standard #: 22VAC40-185-510-A
Description: Based on the inspector's review of a prescription medication, the center failed to ensure parent written authorization for administration was obtained. Evidence: The inspector observed a prescription Albuterol inhaler in the medication lock box for use as needed. The authorization for was not signed by the parent. This is an emergency medication and could be required to be used at any time.

Plan of Correction: The Program Director will have the parent sign the form today during pick up.
