Discussed allergy plans (VENDSUB-000-(5)-006.7 and VENDSUB-000-(10)-035.D.5)
Licensing Inspector arrived at 10:00 AM and departed at 1:00 PM to conduct a Subsidy Health and Safety Inspection in conjunction with a mandated monitoring visit. All applicable areas of "Attachment A" were reviewed. Violations were noted in the areas of children's information and transportation and field trips.
Standard #: VENDSUB-000-035-D-5 Description: Based on review and interviews, it was determined that the vendor did not ensure that the staff have allergy plans available for children during transportation. Evidence: Center management and staff stated that there were not allergy plans available on the vans for school age children that are being transported. Plan of Correction: Management will make copies of allergy plans for transportation notebooks.
Standard #: VENDSUB-000-006-B Description: Based on review of allergy plans, it was determined that vendor did not ensure that there were allergy plans available for two children with food allergies. Evidence: Two children with food allergies (one peanut and one dairy) did not have allergy plans. Plan of Correction: Center management and/or health & safety inspector will contact parents to obtain allergy plans for children.
