Technical Assistance:
Technical assistance today included lists of foods that pose choking hazards for children under age 3, and guidelines on serving small sized items to young children. This included guidelines for fruit and snack items. The responsibility for the appropriateness for age/stage of the food served is up to the center, not the food supplier/contractor. Consultation also included the appropriateness of staff's personal foods accessible to children.

Comments:
COMPLAINT #47405 An unannounced complaint investigation was conducted today. Staff were interviewed; and computer images and records were reviewed. The following standard was reviewed: 560F4: Children under age 3 and foods that pose choking hazards. Staff were interviewed and records were reviewed. Due to the lack of a preponderance of evidence, it does not appear that children were actually offered a choking food (popcorn.) Therefore this complaint is being considered invalid at this time. However, violations were found during the course of the investigation in record keeping, and a Violation Notice was written. Description: During a review of records today, there were items missing from a sample of children's records. Evidence: 1. There were incomplete emergency pick up contacts/addresses for Child # 2 and 4. 3. There was no proof of identification listed for Child # 4, who began 3/2/17. 4. The new center form did not ask parents for employment addresses and three required parent agreements that need to be signed were not listed. 5. There were no immunization records present for two children in care (Child # 4 and 5 - start date of 3/2/17) 6. There were no annual reviews of emergency contacts by parents documented for any of the files of children in care over one year as required.

Plan of Correction: We will correct all of the information, and document the date and signature that the parent reviews the file annually..
