Your time and assistance were appreciated during the unannounced monitoring inspection conducted on April 11, 2017, between the hours of approximately 10:42 a.m. and 2:00 p.m. Fifty-nine (59) children were enrolled and twenty-seven (27) children were in the care of five staff members during the on site inspection. Two/three additional staff members were present and assisted, as needed. Five children's records and twelve (12) staff records were reviewed and/or updated. Three children's medications, two children's topical ointments and accompanying authorization forms were reviewed. Safety equipment (first aid kits, flashlights and radios), posted information (license, licensing inspection documentation, emergency evacuation diagram, emergency telephone numbers, and classroom schedules) and available documents (emergency evacuation drill & shelter-in-place records, attendance logs, infant daily records, injury report forms, insurance verification and local agency report) were observed/reviewed. The daily schedules accurately reflected the activities provided. Lunches were provided by the children's parents. Activities observed on this date included: indoor play (toys and coloring); outdoor play; diapering, restroom and handwashing procedures; lunch; naptime and infant care. Staff/child interactions were observed to be positive, patient and supportive. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Substantial compliance with the licensing standards continues to be maintained. One previously cited violation (inspection date - 10/18/16) was reviewed for compliance: one repeat violation was cited. The exceptions to compliance were noted on the accompanying violation notice. Standard #: 22VAC40-185-160-C Description: Based on review/update of 12 staff records and discussion with staff #1 on April 11, 2017, the program failed to obtain the results of follow-up tuberculosis (TB) screenings, for two staff members, at least every two years from the date of the first initial screening or testing. Evidence: 1. Review of staff #3 and staff #5's records, on this date, revealed that TB updates were not obtained and the results submitted at least two years from the date of the initial screening or testing. 2. Staff #1 confirmed the dates of the initial screenings/tests and the dates of the updated screenings/tests. Plan of Correction: In the future, continued effort will be made to obtain updated TB screening/test results at least every two years from the date of a staff member's initial screening or testing.
Standard #: 22VAC40-185-70-A Description: Based on review and/or update of 12 staff records and discussion with staff #1 on April 11, 2017, the program failed to check and document two or more references prior to the employment of one staff member. Evidence: 1. Review of staff #12's record, on this date, determined that references had not been checked/documented prior to employment. 2. Staff #1 verified that references were not checked and documented prior to the staff member's employment at the facility (hire date - 3/27/2017). Plan of Correction: References for the staff member will be checked and documented.
