Comments:
An unannounced monitoring inspection was conducted from 10:00am-12:30pm. A sample of 3 staff and 3 children's records were reviewed, in addition to 1 medication. Children were observed during nap and playing in centers. Infants were observed sleeping in cribs, being fed and interacting with staff.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure areas and equipment of the center, inside and outside, were maintained in a safe condition. Evidence: In the early pre-k bathroom the metal hardware attached to the toilet plumbing was rusted.

Plan of Correction: Work orders have been placed as of 4/26/17 to have this repaired on or before 5/26/17.

Standard #: 22VAC40-185-450-A
Description: Based on observation the center failed to ensure there was a top and bottom cover used during the designated rest period. Evidence: In the infant-twos room there were 2 children observed on cots during nap that did not have a top cover.

Plan of Correction: Two year old staff have been retrained on what to do if a child does not have a blanket and sheet to sleep with. Management will observe daily for compliance

Standard #: 22VAC40-185-570-A
Description: Based on observation the center failed to ensure high chairs were fastened when in use. Evidence: In the infant room there were 2 babies in high chairs. The chairs had a buckle with no straps in the chair to secure it. Staff confirmed the chairs did have shoulder straps, but don't know where they are.

Plan of Correction: New straps have been ordered as of 4/26/17 and management will monitor daily to ensure compliance. Staff are also being retrained on or before May 5, 2017 on high chair safety.
