Comments:
An unannounced monitoring inspection was conducted beginning at 6:35 a.m. and ending at 7:45 a.m. with the director and one staff. There were twenty-one children in care ranging in age from five years to eleven years of age. Children were observed playing at their choice of centers that included manipulatives, imagination play, board games, and arts/crafts. Hand washing procedures were observed and children that wanted breakfast had cereal and milk. Children were observed cleaning up centers. Five children's records and three staff records were reviewed. Two medications and authorizations were reviewed. Emergency drills were up to date. Description: Based on review of documentation, the center failed to obtain repeat background checks every three years as required. Evidence: Staff #2 most recent criminal records background check was dated 4/22/2014.

Plan of Correction: The office will be contacted for the new background check
