Comments:
Licensing Inspector arrived at 9:00 AM and departed at 2:15 PM. A subsidy health and safety inspection was conducted in in conjunction with a mandated monitoring. All requirements in Attachment "A" were reviewed. Violations were noted in the area of administration.

Violations:
Standard #: VENDSUB-000-004-F
Description: Based on observation and inspection, it was determined that the vendor did not ensure that a current list of children's allergies, sensitivities and dietary restrictions was posted. Evidence: Review of seven staff records revealed that there were two children with food allergies (milk and peanuts) who were not on the allergy list.

Plan of Correction: All children's records will be reviewed and list of allergies, sensitivities and dietary preferences will be updated.

Standard #: VENDSUB-000-006-B
Description: Based on review of seven children's records, it was determined that the vendor did not ensure that all children with a diagnosed food allergy have a written allergy plan to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: There was a child with a milk allergy and a child with a peanut allergy for whom there were no allergy plans.

Plan of Correction: Center management will work with parents to obtain allergy plans.
