Comments:
An unannounced monitoring inspection was conducted on May 1, 2017. At the time of entrance there were 92 children and 6 staff members present. The sample size consisted of 6 children's records. Children were observed eating snack, working on homework, playing board games and video games. Three medications were reviewed. The inspection began at approximately 3:00 p.m. and ended at 6:00 p.m. Areas of non-compliance are noted on the violation notice. The inspection results and violations cited were reviewed and verified with the center director. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return a signed copy to the licensing office within 5 days of receipt. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a record review and staff interviews, the licensee did not ensure each child has documentation of a physical examination conducted by or under the direction of a physician in their record within one month of attendance. Evidence: During the inspection on May 1, 2017, the record for child #6 did not include documentation of a physical and the first date of attendance was 9-6-2016.

Plan of Correction: The Center Supervisor will ensure each child has completed documentation on site to include proper physical examination paperwork.

Standard #: 22VAC40-185-260-A
Description: Based on a documentation review and staff interviews, the licensee did not ensure the center had obtained an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection on May 1, 2017, the most recent fire inspection was dated 4-27-16 and had not been updated annually.

Plan of Correction: The Center Supervisor will ensure an updated annual fire inspection is obtained and posted.

Standard #: 22VAC40-185-510-N
Description: Based on a medication review and staff interviews, the licensee did not ensure that when an authorization for medication expires, the parent was notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. Evidence: During the inspection on May 1, 2017, there were two medications (Epi-pen and Albuterol) that had expired authorization as of April 11, 2017. The medication had not been disposed of by the center or sent home to the parent by the end of the 14 day time frame.

Plan of Correction: The Center Supervisor will ensure all expired medications and those with expired authorization forms will be sent home or disposed of properly.
