Comments:
A self-reported incident was reported to the Fairfax Licensing Office by center management. Based on staff interviews and written statements, the preponderance of evidence supports the allegation and violations have been cited. Areas of non-compliance are identified in this report. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on staff interviews, staff did not follow the center procedures to take a head count when exiting the playground and before entering the building. Evidence: On 4/19/2017 at approximately 4:30 Staff #1 and Staff #2 exited the playground area and entered the center without counting the number of children in care to ensure all were accounted for in their group.

Plan of Correction: Review CCCC procedure for head count. Changed procedure for pm outdoor activities-Inf-2 year old dismissal from classroom only,

Standard #: 22VAC40-185-340-A
Description: Based on staff interview and written statements, staff did not ensure care and protection of a child in their direct care. Evidence: Child #1 was left on the playground when the group left at approximately 4:30. The child was seen by a parent and was also seen by Staff #3 inside the center. Staff #1 immediately went back to the playground and bought the child back into the center at approximately 4:32. For two minutes the child was outside, without a center staff member being able to offer immediate assistance if needed.

Plan of Correction: Review CCCC procedures with staff 1 and 2. Verbal counseling documented in personnel file.

Standard #: 22VAC40-185-340-F
Description: Based on staff interviews and written statements, a child under the age of 10 years was not always in sight and sound supervision of staff. Evidence: Child #1 was left unattended on the center playground for approximately 2 minutes and not in sound supervision of direct care staff.

Plan of Correction: Thorough review of incident resulted in program dismissal changes (see above)-update parent and employee handbook.
