Comments:
An unannounced monitoring inspection was conducted today from 11:20am-12:20pm. There were 41 primary children directly supervised by 4 staff. The physical plant, staff records were current from previous inspection and were not reviewed, 4 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed cleaning up their activities, circle time and preparing to go outside. There was an sufficient supply of books, toys, and materials for the children. Description: Based on review, the facility failed to ensure that children's records were maintained with the required information. Evidence: Reviewed 4 children's records and found that CH1 did not have documentation of a complete address for their second emergency contact.

Plan of Correction: I will email the parent.
