Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-185-(6)-420-E (Parent involvement); 22VAC40-185-(6)-430 (Equipment and materials)

Comments:
An unannounced monitoring inspection was conducted on 5/8/17 from 9:40am - 12:15pm. During the inspection there were 42 children ages five months to five years old in care with 11 staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms, during rest period, playing outside, and eating lunch. Records were reviewed for five children and three staff. There was no medication at the facility. Emergency procedures and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of three staff records, it was determined that the facility did not ensure that each staff records contain all of the required items. Evidence: 1. The record for staff #3 did not contain the name, address, and telephone number of a person to be contacted in an emergency. 2. Staff #4 (Prgram Director) confirmed the record for staff #3 was not complete.

Plan of Correction: The facility responded: Corrected during the inspection. All new staff will document an emergency contact in their record prior to working with children.
