22VAC40-185 STAFFING AND SUPERVISION.

Comments:
Today an investigation into a self-reported incident was conducted as a result of a child left outside on the playground. After interviewing staff, reviewing the incident report and face to name recognition clipboard, it was determined that the child was left unaccounted for and not within sight and sound supervision for approximately 6-9 minutes. Violations were cited.

Standard #: 22VAC40-185-40-E
Description: Based on interviews with Administrators and staff and review of the center's video recording, staff failed to comply with the Center's own policies for accounting for children. Evidence: The Center's Face to Name recognition policy requires staff to do a Face to Name count when leaving the classroom, when they get to the playground, when they leave the playground, and again upon returning to the classroom. Staff are required to use the Face to Name form attached to a clipboard, to document and verify that each child is accounted for when reentering the building. Staff are required to call each child?s name to ensure that the child enters the building and are accounted for, and when staff enter the classroom the Face to Name procedure is to be repeated. On April 28, 2017, staff did not implement the Face to Name policy and procedures when leaving the playground at approximately 4:15pm, or when entering the classroom at 4:18pm. The clipboard with the Face to Name form and documentation was later found on the playground.

Plan of Correction: Hopkins House has a zero tolerance policy as it relates to ensuring the safety of all children that are enrolled in our Academy. The three staff members that were responsible for the care of those children were terminated immediately. All staff are trained upon hire and continue to receive ongoing training on how to have effective transitions as they move from place to place.

Standard #: 22VAC40-185-340-A
Description: Based on interviews with Staff and review of the center's video recordings, it was determined staff failed to ensure care, protection and guidance while supervising children. Evidence: On Friday April 28, 2017, while bringing a group of children into the building from the playground, Staff #1, 2 & 3 failed to implement the center's Face to Name recognition procedures to account for all children. The children and staff began entering the building at approximately 4:15pm and entered the classroom at 4:18pm. Child #1's (19 months old) parent arrived at the Center at approximately 4:24pm, and noticed Child #1 standing outside the back door. When the parent went to Child #1's classroom at 4:25pm, staff were unaware that the child was not with the group. Child #1 was left alone and unsupervised outside for at least 6-9 minutes without staff aware that the child was missing.

Plan of Correction: Principal stated that staff was just trained on 4/7/17 on face to name procedures however an updated /more detailed Face to name policy has been updated with clear expectations. A new face to name form was created with additional space for Administrator to sign off when the children leave or return to the building. Staff was retrained and shown the correct way to do face to name and began using to ensure that this does not reoccur.

Standard #: 22VAC40-185-340-F
Description: Based on interviews with Administrators and Staff and review of the center's video recording, it was determined staff failed to provide sight and sound supervision for a child under 10 years of age. Evidence: On Friday April 28, 2017 at approximately 4:13pm the video recording shows Child #1 outside the back door with staff. At 4:15pm the video recording shows Child #1 running from the back door and towards the playground while the rest of the classroom was entering the building. At 4:18pm, the toddler class entered the classroom after leaving the playground. At 4:21pm the video recording shows Child #1 standing outside the back door. At approximately 4:24 pm, the parent found child #1 outside standing at the back door. The staff did not realize Child #1 (19 months old) had been left outside unaccounted and unsupervised until it was brought to their attention by the child's parent. Child #1 was out of sight and sound supervision for at least 6-9 minutes.
