Comments:
This unannounced annual subsidy health and safety inspection was conducted from 9:00 A.M. until 2:30 P.M. At the time of the inspection 52 children were present with 11 staff. The subsidy requirements were reviewed with the director. Three children's record and five staff's records were reviewed and the emergency plan was reviewed. Standard #: VENDSUB-000-012-C
Description: Based on interview, the director failed to have all staff hired prior to January 1, 2017 complete Virginia Preservice Training for Child Care Staff before March 31, 2017.

Evidence:

The director stated all staff hired prior to January 1, 2017 have completed the Virginia Preservice Training for Child Care Staff except staff 6.

Plan of Correction: Staff 6 has been working on completing preservice.

Standard #: VENDSUB-000-012-E
Description: Based on interview, the director failed to ensure all staff hired prior to January 1, 2017 completed CPR and first aid certification by March 31, 2017. Evidence: The director stated six out of 12 staff have completed CPR and first aid certification.

Plan of Correction: All staff employed in October 2016 were certified in the same class. The next class we have found is in June. Will have additional staff attend then.

Standard #: VENDSUB-000-031-B-1
Description: Based on a review of the evacuation procures, the director has failed to update the evacuation procedures to include all the new requirements. Evidence: The current evacuation procedures did not contain accommodations or special requirements for infants, toddlers, and children with special needs and procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child.

Plan of Correction: All procedures are in the process of being reviewed and updated.

Standard #: VENDSUB-000-031-B-2
Description: Based on a review of the shelter-in-place procures, the director has failed to update the shelter-in-place procedures to include all the new requirements. Evidence: The current shelter-in-place procedures did not contain accommodations or special requirements for infants, toddlers, and children with special needs and procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child.

Plan of Correction: All procedures are in the process of being reviewed and updated.
