Technical assistance was provided in the following areas of the standards: 22VAC40-185-(3)-210-D (Program leader qualifications); 22VAC40-185-(5)-340 (Supervision of children)
An unannounced monitoring inspection was conducted on 5/16/17 from 3:15pm - 4:45pm. At the time of entrance there were 64 school age children in care with eight staff. Children were observed completing homework, participating in various activities, eating snack and playing outside. Records were reviewed for five children and three staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.
Standard #: 22VAC40-185-160-C Description: Based on a review of three staff records, it was determined that the facility did not ensure that staff resubmit updated TB results every 2 years. Evidence: 1. The record for staff #1 had documentation of a TB screening dated 3/31/15. 2. Staff #4 (Program Director) confirmed that an updated TB screening had not been received for staff #1. Plan of Correction: The facility responded: Staff #1 will get an updated TB screening.
Standard #: 22VAC40-185-270-A Description: Based on observation, it was determined that the facility did not ensure that all areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. Evidence; 1. Directly outside the door that leads to the playground, there was a hose on the ground running from the building to the fenced construction area. The hose is a tripping hazard for children exiting the facility and entering the playground. 2. Staff #4 (Program Director) confirmed that the hose was a tripping hazard for the children. Plan of Correction: The facility responded: The hose will be removed or covered in a way that alerts students to it's presence and eliminates it as a tripping hazard.
