Evidence: There are areas on the playground for the Village when soil has eroded to the point that it poses tripping hazards and has areas of standing water. These include, but are not limited to, an area outside the door by the restrooms to the playground, around the stepping stones leading from the door listed above to the main playground and the areas near the concrete area outside the private Kindergarten room where the portable basketball hoop was located.

Plan of Correction: Sand and pebbles will be place in all areas where there is soil erosion. Areas surrounding stepping stones wil be dug and leveled.

Evidence: A staff member in the infant classroom did not sanitize the diapering surface between changing two children.

Plan of Correction: Two waterproof barriers were provided to the infant room to use in addition to diaper pad. The Director met with all infant room staff to ensure proper diapering procedures.

Standard #: 22VAC40-185-560-N
Description: Based on inspection of the infant room, it was determined that the facility did not ensure that children were not allowed to eat or drink while walking around. Evidence: A child in the infant room was observed walking around with a sippy cup.

Plan of Correction: The Director met with all infant room staff to develop procedures for better clean-up after snacks and lunch. It was reiterated to staff that children may not eat or drink while walking.
