Standard #: 22VAC40-185-280-B
Description: Based on observation and discussion with staff on May 16, 2017, the program failed to ensure a hazardous substance (air freshener) was kept in a location that prevented access by children. Evidence: 1. Observation in classroom DD, on this date, revealed that a container of solid air freshener was located on the back of the toilet. 2. Staff #1 verified the location of the air freshener.

Plan of Correction: The air freshener was immediately removed. In the future, air freshener will not be kept in areas accessible to children.

Standard #: 22VAC40-185-520-B
Description: Based on observation of two children's topical ointments and discussion with staff on May 16, 2017, program staff failed to label the container for one child's sunscreen with the child's name. Evidence: 1. Observation in classroom AA, on this date, determined that child D's sunscreen was not labeled with the child's name. 2. Staff #8 confirmed the lack of labeling on the sunscreen container.

Plan of Correction: The sunscreen container was immediately labeled with the child's name. In the future, all children's sunscreen containers will be labeled with their names.

Standard #: 22VAC40-185-520-C
Description: Based on observation of two children's topical ointments and discussion with staff on May 16, 2017, program staff failed to label one child's diaper ointment with the child's name. Evidence: 1. Observation in classroom AA, on this date, determined that child B's diaper ointment was not labeled with the child's name. 2. Staff #8 confirmed the lack of labeling on the diaper ointment container.

Plan of Correction: The diaper ointment container was immediately labeled with the child's name. In the future, all children's diaper ointment containers will be labeled with their names.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on review/update of seven staff records and discussion with staff #1 on May 16, 2017, the program failed to obtain a central registry (CPS) clearance before 30 days of employment ended for one employee. Evidence: 1. Review of staff #7's background clearances, on this date, disclosed that a central registry clearance was not included in the staff member's record (employment date - 3/29/2017). 2. Staff #1 confirmed that the background clearance had been submitted, was returned and resubmitted for processing.

Plan of Correction: The background clearance has been submitted for processing and will be obtained.

Standard #: 22VAC40-191-50-A-3-a
Description: Based on review/update of seven staff records and discussion with staff #1 on May 16, 2017, the program failed to ensure an employee did not continue to work after a criminal history record clearance was processed and returned with unsatisfactory results. Evidence: 1. Review of background clearances for staff #7, on this date, determined that the criminal history record clearance contained unsatisfactory results. 2. Staff #1 verified the results of the criminal history record clearance.

Plan of Correction: As per conversation with our state representative, we were specifically informed that the background check would be satisfactory.
