22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted on Thursday, May 25, 2017, from approximately 2:00pm until 6:00pm. Present today were: Infant A - Six children with two staff Infant B - Seven children with two staff Toddlers - (16mos-2 yrs) Ten children with two staff Twaddlers - ( 2-2 1/2 yrs.) Twelve children with two staff Preppers- (2 1/2 - 3 1/2 yrs.) Thirteen children with two staff PreSchool I - (3's) Thirteen children with two staff PreSchool II ( 3's and 4's) Eleven children with two staff PreK - Six children with one staff PreK II - Six children with one staff School age: Fifteen children with one staff Infant children were observed being held, fed, changed and spoken to frequently. Other classrooms were observed during naptime, transition to snack and while preparing for afternoon activities. The children were playing with manipulatives, listening to stories, printing their names, making "peace angels", creating with play dough, coloring and having indoor gross motor activities in the "Make Believe Room" and in the "Theatre/Dance Studio". Lunch: Chicken bites, broccoli, mashed sweet potatoes, milk PM Snack: apple slices and crackers The last practice evacuation drill was conducted 4/28/17. Five children's records, three staff records, and three children's medications were inspected.
Standard #: 22VAC40-185-160-A Description: Based on review of staff records, the center did not ensure each staff received a negative tuberculosis screening (TB) within 21 days of employment. Evidence: Out of three staff records inspected, Staff #1 started working 2/13/17, however, this staff person's documentation for TB screening was dated 5/11/17. Staff #2, began working 2/13/17 and Staff #3, began working 1/30/2017, and do not have documentation in their record of TB screening. Plan of Correction: Per Center: Both staff will be immediately sent for TB screenings. Management will create a checklist to ensure all documentation has been received in allotted time.
Standard #: 22VAC40-185-70-A Description: Based on inspection of staff records, the center did not ensure qualifying documentation for lead teachers was present in the record. Evidence: The record for Staff # 2, listed as a Lead Teacher, does not contain documentation qualifying the individual for a lead teacher position. Plan of Correction: Per Center: We will have the staff person request documentation of education.
Standard #: 22VAC40-185-500-B Description: Based on inspection of the facility, the center did not ensure that disposable diapers were disposed in a storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the diaper touches an exterior surface of the storage system during disposal. Evidence: In the Twaddler and Toddler Classrooms, staff placed soiled diapers in containers that could be operated by foot, however, the staff were opening the containers with their hands. Plan of Correction: Per Center: Corrected during inspection. Staff will be re-trained on appropriate diaper disposal.
Standard #: 22VAC40-185-510-A Description: Based on inspection of medications, the center did not ensure doctor's written authorization was obtained for each prescription medication. Evidence: Out of three medications inspected, medication #1 for a child did not contain written authorization from the physician. Plan of Correction: Per Center: We are going to obtain a medical plan signed by a physician.
