Comments:
An unannounced monitoring inspection was conducted today from 10:15am-12:00pm. There were 35 (Toddlers and Primary) directly supervised by 7 staff. The physical plant, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating snack. Toileting and hand-washing procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-185-60-A
Description: Based on review, the facility failed to ensure that children's records were maintained with the required information. Evidence: Reviewed 5 children's records and found that CH2 did not have documentation of a work number.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility failed to ensure that it areas inside and outside are maintained in a clean, safe and operable condition. Evidence: 1) Observed in the Primary bathroom, under the window, peeling paint that was pulling away from the wall. 2) Observed an approximately 6 inch knife on the counter in the toddler classroom.

Plan of Correction: Will talk to the church about the peeling paint in the bathroom. Knife corrected during the inspection.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility failed to ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) Observed 2 bottles of Meyers Clean Day spray accessible to children in the toddler room. 2) Observed hydrogen peroxide spray accessible to children on a shelf in the toddler room. 3) Observed miracle grow and rubbing alcohol in an unlocked cabinet in the Primary room, above the sink.

Description: Based on observation, the facility failed to ensure that the most recent inspection notice was posted. Evidence: observed that the most recent inspection notice was not posted, but was in the office.
