An unannounced complaint inspection was conducted on May 16, 2017 in regards to staffing and supervision. An inspection was conducted at the facility, interviews were conducted with staff and collateral witnesses and documentation was obtained. Based on staff interviews and a review of documentation, there is a preponderance of evidence to support the allegations therefore this complaint has been determined to be VALID. Description: Based on a review of documentation and staff interview, the licensee did not ensure that ratios were maintained for children 16 months old to two years: one staff member for every five children. Evidence: During the inspection conducted on May 16, 2017, it was verified that on May 10, 2017 from approximately 8:15am-8:23am there were 11 children with two staff members. The youngest child in the group was in the 16 month to two year old age range.

Plan of Correction: We will be sure to monitor children every 10 minutes during arrival and departure to ensure we maintain appropriate ratios. Based on the evidence, the youngest child was in the 16 month to two year old group but was turning two years old on May 11, 2017.
