Technical assistance was provided in the following areas of the standards: 22VAC40-185-(4)-330 (Play areas); 22VAC40-185-(7)-510 (Topical Ointments)
An unannounced monitoring inspection was conducted on 6/6/17 from 10:24am - 2:20pm. During the inspection there were 55 children ages three months to five years old in care with 12 staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms, during rest period, and eating lunch. Records were reviewed for five children and 19 staff. Medication, emergency procedures, and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.
Standard #: 22VAC40-185-140-A Description: Based on a review of five children's records, it was determined the facility did not ensure that each child has a physical examination within one month of the child's attendance. Evidence: 1. The record for child #3 (date of enrollment 5/1/17) did not include a physical examination. 2. Staff #20 (Program director) confirmed that there was no physical examination in the record for child #3. Plan of Correction: The facility responded: Corrected during the inspection, parent of child #3 was called and sent over a copy of the current physical examiantion. All children enrolled will have a physical examination in their record within 30 days of their start date.
Standard #: 22VAC40-185-160-A Description: Based on on a review of 19 records, it was determined that the facility did not ensure that each staff member shall submit documentation of a negative tuberculosis screening no later than 21 days after employment. Evidence: 1. The record for staff #12 (date of hire 2/18/17) contained a TB screening that was dated 4/26/17. 2. Staff #20 (Program Director) confirmed that the TB screening was received more than 21 days after staff #12 began employment. Plan of Correction: The facility responded: All new staff will have a TB screening within 21 days of employment.
Standard #: 22VAC40-185-440-E Description: Based on observation, it was determined that the facility did not ensure that there was 12 inches of space between occupied cots. Evidence: 1. There were two cots in the Three's classroom and two cots in the Four's classroom that did not have 12 inches of space between them. 2. Staff #20 (Program Director) confirmed that there was not 12 inches of space between all of the occupied cots in the center. Plan of Correction: The facility responded: Corrected during inspection. All staff will be re-trained to ensure there is 12 inches of space between all occupied cots.
Standard #: 22VAC40-185-510-N Description: Based on a review of medication, it was determined that the facility did not ensure that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Evidence: 1. There was a Ventolin inhaler for child #6 that had an authorization for medication that expired on 5/16/17 that was in the medication storage box. 2. Staff #20 (Program Director) confirmed that the medication for child #6 had not been returned to the parent within 14 days of the expiration of the medication authorization. Plan of Correction: The facility responded: The medication will be returned to the parent. All medication will be returned within 14 days of expiration of the medication authorization.
