32.1 Report by person other than physician
70 Children were in care during the inspection. The age range of the children was four months to nine years old. Children were observed during out side time, snack time, free play. Four staff are current in MAT. Inspection time 2:00pm-5:00pm
Standard #: 22VAC40-185-280-B Description: Based on observation not all hazardous substances were kept in a locked place. Evidence: In the school age room there was bottles marked bleach water stored in a unlocked cabinet and there was a box of Borax stored on a cabinet. The borax had keep out of reach of children warning label. Plan of Correction: We will remind all staff to keep all hazardous substances are kept in a locked area.
Standard #: 22VAC40-185-370-1 Description: Based on review there was no documentation of infants that cannot roll over are receiving 30 minutes of tummy time. Evidence: There was no documentation of younger infants receiving 30 minutes of tummy time. Plan of Correction: We will remind all infant teacher's to record tummy time.
Standard #: 22VAC40-185-500-A Description: Based on observation staff did not was hands after changing a diaper and the children's hands were not washed after a diaper change. Evidence: Staff in the infant room did not wash their hands after changing a diaper. The staff did not wash the children's hands after having a diaper change. Plan of Correction: We will retrain all infant teachers on diaper changing.
Standard #: 22VAC40-185-500-B Description: Based on observation, 1. staff are not using their foot to open the foot operated diaper pail. 2. The diaper changing pad was not washed or sanitized after a diaper was changed. Evidence: 1, staff in the infant room used their hands to open the diaper pail. 2, the staff did not clean and sanitize the diaper changing pad after use. Plan of Correction: We will retrain all infant staff on diaper changing procedures.
Standard #: 22VAC40-185-510-A Description: Based on review of medication not all had a current ten day parent authorization. Evidence: Child A had a medication stored in the center and the authorization is dated 2/23/17. Plan of Correction: We will send home the medication today.
