An unannounced monitoring inspection was conducted at the facility on June 9, 2017 from approximately 9:55am- 12:30 pm. Upon entrance there were thirty eight children with 9 staff. Additional staff was present during the inspection. The licensing inspector observed the children freely playing, eating snack, arts and craft activity and outside play. The sample size consisted of 3 children records and 3 staff records. Areas of non-compliance was reviewed with the director.Medication is not given at the center. Exit interview was conducted with the program director.
Standard #: 22VAC40-185-270-A Description: Based on observation, the licensee did not ensure the inside and outside , was maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on June 9, 2017, the following was observed on the outside area on the playground: - The wooden boarder around the sand box is deteriorating and causing splintering. - The fence panel on the outside of the infant and toddler playground by the larger playground, a panel has detached and has caused two nails to be exposed . - A red broken sand bucket on the playground. Plan of Correction: The sand box was removed. The frame panel was replaced. The bucket was removed. Director will inspect playground weekly for problems.
Standard #: 22VAC40-185-280-B Description: Based on observation, the licensee did not ensure all hazardous substances were kept in a locked place. Evidence: During the inspection conducted on June 9, 2017 , the licensing inspector observed in the girls bathroom on a shelf all purpose cleaner labeled "Keep out of reach of children" and " Caution ". Plan of Correction: The cleaner was removed. Staff was reminded where to store items.
Standard #: 22VAC40-185-330-B Description: Based on observation and inspection of the playground, the licensee did not to ensure that the required fall zone encompasses sufficient area. Evidence: During the inspection conducted on June 9, 2017, the green and pink moveable slide had a 1ft and 4in fall zone in front of the slide and the pirate play piece with climbing apparatus and slide was had 2ft fall zone. All slide and climbing apparatus should have a fall zone of 6 feet. Plan of Correction: Moveable slides were removed . All fall zones were checked . Director will inspect weekly to ensure fall zone remain.
Standard #: 22VAC40-185-500-B Description: Based on observation, the licensee did not ensure the diapering surface was used only for diapering or cleaning children and the diapering surface was a nonabsorbent surface for diapering. Evidence: During the inspection conducted on June 9, 2017,the following was observed: -In the Busy Bee classroom there were 2 tears on the diapering pad. -In the Caterpillar classroom there were tears on the diapering pad. Plan of Correction: Diaper pad replaced . Staff was reminded to replace any with torns.
