Comments:
An unannounced monitoring inspection was conducted on June 6, 2017. At the time of entrance there were 111 children and 20 staff members present with children. Additional staff were on site for administrative and meal preparation responsibilities. The sample size consisted of 6 children's records and 4 staff records. Children were observed eating morning snack, playing outside and participating in circle time. Previous violations were reviewed. The inspection began at approximately 8:25 a.m. and ended at approximately 11:20 a.m. Areas of non-compliance are identified on the violation notice. The results of the inspection and violations cited were reviewed and verified with the center director during the exit interview. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return it to the licensing office within 5 calendar days of receipt. The provider?s responses for the "Description of Action To Be Taken" was not received as of June 20, 2017 and will not appear on this" Violation Notice". Standard #: 22VAC40-185-60-A
Description: Based on observation, the licensee did not ensure the center maintained and kept at the center a separate record for each child enrolled which contains all the required following information Evidence: During the inspection on June 6, 2017, six children's records were reviewed and the following information was not available for review; 1) The records for child #1, #2, #3, #5 and #6 did not include information regarding other programs that the child may or may not attend. 2) The record for child #2 did not include information regarding a second emergency contact name, phone number or address. 3 ) The records for child #3 and child #5 did not include information regarding previous childcare. 4) The record for child #6 did not include the employment phone number for one of the parent's listed.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the licensee did not ensure areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: During the inspection on June 6, 2017, the following areas were observed and determined to pose a risk to the children in care; 1) On the toddler playground near the shaded cover there was a white plastic pipe approximately 5 inches in diameter protruding from the ground and is a tripping hazard. 2) There were 2 outside metal window frames that were observed to be in an unsafe condition. The windows face the outdoor playground areas. The window screens have been removed from the metal frames and the frames have sharp metal edges that are exposed.

Description: Based on observation, the licensee did not ensure medication was labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. Evidence: During the inspection on June 6, 2017, there were 3 medications observed that were not labeled with the child's name. The center director verified the following medications belonged to the following children. 1) Tylenol belonged to child #3 2) Benedryl belonged to child #1 3) Benedryl belonged to child #2

Standard #: 22VAC40-191-60-B
Description: Based on a record review, the licensee did not ensure all employee of a licensed or registered child welfare agency must not be employed or provide volunteer service until the agency has the persons completed sworn statement or affirmation. Evidence: During the inspection on June 6, 2017, the record for staff #4 did not include a sworn statement of affirmation. The date of hire for staff #4 was 2-13-2017. Staff #4 was observed working during the inspection.
