Comments:
The licensing inspector conducted an unannounced complaint inspection on June 15, 2017 in response to a complaint that was received by the licensing office on 6/2/17. Observations were completed, documentation reviewed and interviews were conducted with staff relating to allegations concerning staff-child ratios, incident reporting and supervision. There was a preponderance of the evidence to support the allegations. The complaint was found to be valid. Please complete the columns for "description of action to be taken" and "date to be corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 5 business days of receipt. Complaint related: No
Description: Based on a review of reports at the facility, the facility did not ensure that a report on a child was made accessible for two years after termination of services. EVIDENCE: The facility did not have a copy of an incident report written for child #1, termination date 5/19/17, regarding an incident that occurred on 5/19/17.

Plan of Correction: Facility will keep all incident reports for the required 2 year period on file.

Description: Based on interviews at the facility, the facility did not ensure that children in the toddler class were within sight supervision of staff at all times. EVIDENCE: On 5/18/17, the staff person in the toddler room, left toddlers in their cribs and took a child to the adjacent infant room to change a diaper. While in the infant room, the staff person was unable to see the children that remained in the toddler room.

Plan of Correction: Counsel staff and train staff to remain with children at all times until assistance is available to ensure that all children are within sight and sound.

Description: Based on documentation at the facility, the facility did not ensure that the ratio was maintained at 1:4 in the infant class. EVIDENCE: After reviewing attendance records for 5/18/17, it was determined that the infant class was out of ratio. There were 6 infants with only one staff person.

Plan of Correction: Facility will counsel all employees on the ratio, provide training and have enough staff on duty to comply.

Description: Based on interviews at the facility, the facility did not ensure that a child's parent was notified of a minor injury by the end of the day. EVIDENCE: Per staff person #1:On 5/19/17 child#1 fell and sustained a minor bruise. The child's parent was not notified by telephone and the incident report was mistakenly sent home with another child. The miscommunication resulted in the parent not receiving notification of the child's injury by the end of the day.

Plan of Correction: Facility will notify parents verbally and written by end of day of minor and major incidents. Copies of reports, will be signed by parent when received.
