Comments:
An announced other inspection was conducted on June 27, 2017 to review staff records. The sample size consisted of four staff records. Areas of noncompliance are identified on the Violation Notice.

Violations:
Standard #: 22VAC40-185-210-A-4
Description: Based on record review, the licensee did not ensure that program lead staff have six months of supervised programmatic experience. Evidence: During the inspection conducted on June 27, 2017, Staff #4 who was identified in the record as a recreation leader (hire date 4/14/17) did not have any documented supervised programmatic experience.

Plan of Correction: We will clearly notate on the staff file that the employee starts working for their first six months in the aid capacity. Once the required six months of experience has been accrued the record will reflect the recreation leader position.
