22VAC40-191 Background Checks (22VAC40-191)
An announced record review was conducted on June 27, 2017. A sample of three staff records were reviewed. Areas of non-compliance are identified on the violation notice. The results of the inspection and violations cited were reviewed and verified with the Recreation Program Coordinator during the exit interview.
Standard #: 22VAC40-185-210-A-4 Description: Based a record review and staff interviews, the licensee did not ensure the program leaders were had at least six months of supervised programmatic experience. Evidence: During the inspection on June 27, 2017, one out of three records reviewed did not include documentation to indicate staff #3 had the required six months of supervised programmatic experience. Staff #3 was hired on 12-16-2016 and their designated job title/position is program leader. The Recreation Program Coordinator verified the job position, date of hire and lack of documented supervised experience. Plan of Correction: We will clearly notate on the staff file that the employee starts working for their first 6 months in the aide capacity. Once required six months of experience has been accrued the record will reflect the recreation leader position.
Standard #: 22VAC40-191-60-C-1 Description: Based a record review and staff interviews, the licensee did not ensure the licensed child welfare agency had an original criminal history record report (CRC) within 30 days of employment or employment shall be denied. Evidence: During the inspection on June 27, 2017, one out of three records reviewed did not include documentation of an original CRC obtained within 30 day or employment was denied. Staff #3 was hired on 12-16-2016 and the original CRC is dated 5-12-2017. Employment for staff #3 had not been denied as verified by the Recreation Program Coordinator. Plan of Correction: Moving forward we will deny employment if the background checks have not been returned within 30 days.
