Standard #: 22VAC40-185-340-A Description: Based on interviews, review of injury reports and photos, the facility did not ensure the care, protection and guidance of children while being supervised by staff. Evidence: 1. Documentation on child #2's enrollment application revealed that child #2 had a history of biting. 2. Injury report dated 4/25/17 documented that a child (Child #2) scratched the face of Child #1. 3. Pictures of the 4/25/17 injury showed broken skin on Child #1?s face. 4. Injury report dated 5/09/17 documented that Child #2 bit the forehead of Child #1. (Injury occurred 05/08/2017) 5. A picture of the 5/08/17 injury showed a bite mark with broken skin on Child #1?s forehead. 6. Injury report dated 6/12/17 documented that Child #2 bit the back of Child #1. (Injury occurred on 06/08/2017) 7. A picture of the 06/08/2017 injury showed a bite mark on Child #1's back. 8. Injury report dated 6/14/17 documented that another child (Child #2 ) scratched the face of Child #1 and also, bit Child #1?s arm. 9. Injury report dated 6/16/17 documented that another child (Child #2) bit the back of Child #1. 10. Adequate prevention plans were not put in place to increase supervision and reduce injuries. Injury reports stated that child #2 would be watched closely and children would be separated. 11. Guardian #3 stated that child #1 and child #2 were separated briefly and then were returned to the same classroom. Plan of Correction: Per the owner: The child (child #2) services were terminated on 06/16/2017. We are working on a biting policy and procedures for staff.
Standard #: 22VAC40-185-550-L Description: Based on review of injury reports and interview, the facility did not notify the parents by the end of the day of any known minor injuries. Evidence: 1. Guardian #3 stated that she saw a bite mark on the back of child #1 on 06/8/2017. 2. Guardian #1 stated she picked up child #1 on 06/09/2017 and request information regarding the bite mark. 3. An injury report was not completed until 06/12/2017 at which time it was provided to Guardian #1. Plan of Correction: Per the owner: Point of contact will be Guardian #1 and Guardian #3 for all communications regardless of who picks the children up. During the time period of the injury several different adults were picking the children up.
Standard #: 22VAC40-185-550-M Description: Based on review of injury reports, the facility did not ensure the reports contained all the required information. Evidence: 1. Injury report #2: dated 05/09/2017 did not have the time of the injury documented. 2. Injury report #3: dated 06/12/2017 did not contain the time of injury, date and time when parent was notified, how parent was notified, second staff signature, nor parent signature. 3. Injury report #4: dated 06/14/2017 did not contain the date and time parent was notified, second staff signature, nor parent signature. 4. Injury report #5: dated 06/16/2017 did not contain the time parent was notified. Plan of Correction: Per the owner: I have review with staff that all injury reports must be completed with all information documented at time of injury.
