Comments:
An unannounced renewal inspection was conducted on July 10, 2017, between the hours of approximately 10:30 a.m. and 5:10 p.m. Eighty-three (83) children were enrolled and fifty-nine (59) children were in the care of nine staff members, in three classrooms, during the on site inspection. One additional staff member was present and assisted, as needed. Six children's records and six staff records were reviewed and/or updated. One child's medication, three children's diaper ointments and two children's sunscreen products were observed/reviewed on this date. Emergency equipment (first aid kit, flashlight and battery-operated radio), posted information (license, inspection documentation, weekly menu, emergency plan, emergency telephone numbers and classroom schedules) and available documents (emergency evacuation drills/shelter-in-place drill records, attendance records, infant daily records, injury report forms, emergency plan and local agency inspection reports) were observed/reviewed. The daily schedule accurately reflected the activities provided. Packed lunches were provided by the children's parents for the school-age children and lunches for the infants and toddlers were provided by the program. Observed staff/child interactions were positive and supportive of children's growth and development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Program observation was completed. Activities provided on this date included: indoor play; preparation for school-age field trips to the caverns and the park; lunch; naptime; infant care and arrival/departure of children. Refer to the accompanying violation notice for the one exception to licensing standards compliance cited during this inspection. Description: Based on review of six staff records and discussion with staff on July 10, 2017, the program failed to obtain documentation that two or more references as to character and reputation as well as competency were checked before employment. Evidence: 1. Review of staff #6's record, on this date, determined that documentation of two reference checks was not on file (hire date 6/19/2017). 2. Staff #2 and #3 confirmed the lack of reference check documentation in staff #6's record.

Plan of Correction: Two references were immediately obtained and documentation was placed in the staff member's record (copies of the references).
