Comments:
An unannounced Safety, Health and Subsidy inspection was made on 7/18/17 at 10:15 a. m. and concluded at 2:30 p. m. The inspector met with the staff and regulations were reviewed. The staff-to-child ratios were observed: Infants #1: Four infants with two staff. Infants were having free play and one infant was napping. Infants #2: Six infants with two staff. Infants were having free play and two infants were napping. Toddler #1: Five children with two staff. Children and staff were in having free play. Toddler #2: Six children with two staff. Children and staff were having free play. Early Preschool #1 (Two Years): Eight children with two staff. Children and staff were having interest centers. Early Preschool #2 (Two Years): Eight children with two staff. Children and staff were having interest centers. Preschool #1 (Three Years): 15 children with two staff. Children and staff were having interest centers. Preschool #2 (Three Years): 14 children with two staff. Children were having a story. Pre-K #1(Four Years): 16 children with two staff. The children were having a variety of interest centers. Pre-K #2(Four Years): 13 children with two staff. The children were having interest centers. School-Age Class 18 children with two staff. Children were having interest centers. Age and stage appropriate activities were observed in all classrooms today. Four classroom observations were conducted. Conversations were held with several staff and children. Ten staff records were reviewed. Ten children records were reviewed. Medication procedures were reviewed for five children. The last monthly evacuation drill was documented on 6/27/17.

Description: Based on review of a child's record the center did not have all required information on file. Evidence: On 7/18/17, child #1 (with an egg allergy) did not have an allergy plan in place from the child's physician.

Plan of Correction: Have contacted the parent and physician to get the allergy plan in place as soon as possible.

Standard #: VENDSUB-000-012-E-1
Description: Based on review of staff training the center did not have all staff trained in CPR. Evidence: On 7/18/17, staff #1 (employment date 4/17/17) did not have CPR. CPR training is required within 30 days of hire.

Plan of Correction: Will schedule a CPR class for staff #1 as soon as possible.

Standard #: VENDSUB-000-012-E-2
Description: Based on review of staff training the center did not have all staff trained in First Aid. Evidence: On 7/18/17, staff #1 (employment date 4/17/17) did not have First Aid Training. First Aid training is required within 30 days of hire.

Plan of Correction: Will schedule a First Aid class for staff #1 as soon as possible.
