Comments:
An unannounced monitoring visit and facility tour was conducted from 11:30am-1:25am. At the time of inspectors' arrival there were 19 preschool aged children in care with 2 staff members. Children were observed participating in free play, lunch, nap, transitioning between activities, and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Medications are not being administered currently. Injury reports were reviewed. Areas of noncompliance are detailed on the violation notice and were discussed with the administration staff at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review it was determined the center did ensure staff resubmit TB test results every 2 years from the date of the first initial screening or testing. Evidence: 1. The record for staff #1(hired 8/27/08) did not contain documentation of a current TB test or screening. The last documentation was dated 7/10/15. 2. The record for staff #4 did not contain documentation of a current TB test or screening. The last documentation was dated 6/12/15.

Plan of Correction: Staff will get a updated TB tests or screenings.

Standard #: 22VAC40-185-60-A
Description: Based on record review it was determined that teh center did not ensure that each child's record contained all of the required components. Evidence: 1. The record for child #2(enrolled 9/6/16) did not contain the complete address for one of the two required emergency contacts. 2. The record for child #3(enrolled 7/6/17) did not contain the complete addresses for the two required emergency contacts. 3. The record for child #5(enrolled 1/3/17) did not contain the complete address for one of the two required emergency contacts.

Plan of Correction: Director will request information from parent's at pick up.

Standard #: 22VAC40-185-70-A
Description: Based on record review it was determined the center did not ensure that each staff record contained all of the required components. Evidence: The records for staff #1-5 did not contain the required emergency contacts for employees.

Plan of Correction: The staff records will be updated to include emergency contacts.

Standard #: 22VAC40-185-90--A
Description: Based on record review it was determined the center did not have a written agreement between the parent and the center signed by the first day of the child's attendance. Evidence: The record for child #2(enrolled 9/6/16) did not contain the required signed parental agreements.

Plan of Correction: Director will request that parent's sign the agreements at pick up.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined that the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. There was a 6" sharp knife in the unlocked drawer in the kitchen. 2. There was an opened box of vinyl gloves and gallon ziploc bags on the bottom shelf in the kitchen. -The entrance door to the kitchen is in the pre-k room and was unlocked during this inspection making it accessible to the 19 preschool aged children in care. 3. There were two pairs of sharp teacher scissors, hand sanitizer, and a spool of clear plastic string at child level in the unlocked teachers closet in the preschool room. 4. There was an 2x4" area of peeling paint on the door to the boys rest room. 5. There was a very strong urine odor in the boys rest room and urine on the side of the toilet.

Plan of Correction: Items were moved and made inaccessible to children in care during this inspection. The bathroom will be repainted. The bathroom is cleaned daily.

Standard #: 22VAC40-185-420-E-3
Description: Based on record review it was determined that the center did not request at least annually documented parent confirmation that the required information in the child's record is up to date. Evidence: The record for child #4(enrolled 1/5/15) did not contain documentation of annual review to ensure that the information in the record is up to date.

Plan of Correction: Director will request from parent at pick-up.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on record review it was determined the center did not ensure that each employee has updated criminal history record checks before three years since the date of the most recent criminal history record check. Evidence: The record for staff #1(hired 8/27/08) did not contain an updated criminal history check. The available documentation was dated 7/7/14.

Plan of Correction: A request for an updated criminal history record will be submitted.
