Comments:
An unannounced Safety, Health and Subsidy inspection was made on 7/24/17 at 9:30 a. m. and concluded at 2:00 p. m. The inspector met with the Program Director and Subsidy Requirements were reviewed. The following staff-to-child ratios were observed: Bobcats: (Rising Kindergarten-First Grade) There were 21 children with three staff. The children were engaged in Arts and Crafts under the pavilion. The children were making a jelly fish craft. Wolves: (First-Second Grade) There were seven children with one staff. The children were practicing an "Under the Sea" skit in the Theatre Area. Bears: (Third-Fourth Grade) There were 17 children with two staff. The children and staff were practicing a chant. Eagles: (Fourth-Fifth Grade) There were eight children with two staff and one lifeguard. The children were preparing to put on life jackets prior to getting into the boats. Gators: (Fifth Grade and older) There were were 22 children with three staff. The children were in the Archery area. Seven children records were reviewed. Four medications reviewed. Five staff records were reviewed. Observations were conducted in all camp areas. Conversations were held with several staff and children.

Description: Based on review of children records the center did not have all required information on file. Evidence: On 7/24/17, child #1 and #2 who both have a food allergy listed in their records did not have the required written allergy plan from the child's physician.

Plan of Correction: Ask the parents to provide us with a written allergy plan for their children.

Standard #: VENDSUB-000-031-B-3
Description: Based on review of the Emergency Preparedness Plan the center did not have all required plans available. Evidence; On 7/24/17, the Emergency Preparedness Plan did not include a plan for Lock Down and reunification specific to this center.

Plan of Correction: We will talk to Regional Director to see where were are with developing the Lock Down and reunification plans
