The renewal license cannot be processed until the application is received in our office.

Comments:
An unannounced renewal inspection was conducted on July 24th 2017 between 10:30am and 1:00pm. There were 21 children in the direct care of 5 staff members and 3 volunteers. Children were observed having water play, playing on the playground equipment, building, and sharing. Staff were observed working with children to resolve sharing issues. A sampling of 5 staff records, children's records, the fire and health inspections, the physical space, and supervision were observed and inspected. Standard #: 22VAC40-185-160-C
Description: Based on record review, 2 of 5 staffing records reviewed did not have resubmitted tuberculosis test results every 2 years from the date of the first initial screening. Evidence: Staff #4 last had a tuberculosis screening dated 4/24/2014, and staff #5 last had a tuberculosis screening 11/18/2014

Plan of Correction: We will go get the test completed tomorrow.

Standard #: 22VAC40-185-60-A
Description: Based on record review 2 of 3 children's records reviewed were incomplete. Evidence: Child # 2 and Child #3 records do not contain documentation of 2 emergency contact addresses.

Plan of Correction: I will ask the parents to complete the form.
