Technical Assistance:
*Staff under the age of 18 must be under the sight and sound supervision of an adult staff member. **VA Preservice training can be counted as annual training one time. Please notify your inspector of suspension or termination of services for more than 24 hours as a result of an emergency. Please notify your inspector ?preferably in writing ? of any serious injuries sustained at the facility. (See serious injuries in the Definitions section on pg. 3)

Comments:
An unannounced subsidy health and safety inspection (SHSI) was conducted with the center director and center staff. A licensing inspection was also conducted on this date (see that report for details). There are currently 30 staff employed by the facility. Staff still need documentation of the mandatory VA Preservice Training and current certification in first aid and CPR. There are six children with diagnosed food allergies currently enrolled - please get a written allergy action plan from the physician. The emergency preparedness plan was reviewed for reunification plans and lockdown procedures. Reminder: evacuation drills are required monthly, shelter in place drills are required twice a year, and a lockdown drill is required once a year. Lockdown drills are to isolate children and staff from a security threat; shelter in place is to protect children from a natural or manmade threat. Standard #: VENDSUB-000-006-B-11
Description: Based on record review and staff interview, the vendor failed to have a written allergy care plan for each child diagnosed with a food allergy, to include instructions from a physician regarding the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: there are six children with diagnosed food allergies who do not have a written allergy action plan from the physician.

Plan of Correction: Center staff will request this information from the parents.

Standard #: VENDSUB-000-012-B
Description: Based on review of 30 staff training files, the facility failed to have documentation of Virginia Preservice Training on file for each staff within the first 90 days of employment. Evidence: Thirteen of 30 staff do not have documentation of Virginia Preservice Training on file.

Plan of Correction: Staff will get the required 10 hours of training as soon as possible.

Standard #: VENDSUB-000-012-E-1
Description: Based on review of 30 staff training files, the facility failed to have documentation of CPR training on file for each staff within 30 days of hire. Evidence: Twenty two of 30 staff did not have documentation of CPR training on file.

Plan of Correction: Classes will be scheduled for staff to get the CPR training as soon as possible.

Standard #: VENDSUB-000-012-E-2
Description: Based on review of 30 staff training files, the facility failed to have documentation of current first aid training on file for each staff within 30 days of hire. Evidence: Twenty two of 30 staff did not have documentation of first aid training on file.

Plan of Correction: Classes will be scheduled for staff as soon as possible.
