Comments:
An unannounced subsidy health and safety inspection was completed on 7/27/17 with the licensing renewal inspection.

Violations:
Standard #: VENDSUB-000-012-B
Description: REPEAT VIOLATION Based on record review, the center failed to ensure that four of four staff completed the required Virginia Pre-service training which is required within 90 days of employment or subsidy approval. Evidence: 1. Staff #1,3,4,5 with hire dates of 6/21/12, 10/10/16, 6/22/15, and 8/29/16 had not completed the 10 hour VA Pre-service training.

Plan of Correction: All staff will complete the VA Pre-service training.

Standard #: VENDSUB-000-012-E
Description: REPEAT VIOLATION Based on record review, the center failed to ensure that one of five staff had current first aid and CPR training as required within 30 days of employment. Evidence: 1. Staff #4 who had a hire date of 6/22/15 did not have current first aid and CPR certification. Staff #4 last certification was May 2015 which was current for two years.

Plan of Correction: The child care director will have all staff including staff #4 complete first aid and CPR training.

Standard #: VENDSUB-000-031-B-3
Description: REPEAT VIOLATION Based on interview with the program director via phone, the center failed to ensure that there were written lock down procedures. Evidence: 1. There were no written lock down procedures with reunification.

Plan of Correction: The child care director will develop lock down procedures with reunification.
