An unannounced Subsidy Health and Safety Inspection was conducted on July 31, 2017 from 9:06 a.m. until 12:50 p.m. At the time of entrance there were 7 children and 3 staff members were present with children. There were additional staff on site for administrative purposes. The sample size consisted of 5 children's records and 5 staff records. Children were observed preparing freely playing and dancing. Areas of non-compliance are identified on the violation notice. The results of the inspection were reviewed with the site manager. Standard #: VENDSUB-000-005-B Description: Based on observation and interviews, the vendor did not ensure that for each group of children there was a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: During the inspection on July 31, 2017, there was not a hard copy of daily attendance for the children in room 13. There were 5 children observed in room 13 during the inspection. Plan of Correction: The site manager placed the written in the room #13 today and will educate opening staff of new procedure.
Standard #: VENDSUB-000-031-B-1 Description: Based on observation and interviews, the vendor did not ensure there were procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child during an emergency to include evacuation, shelter-in-place and lockdown emergencies. Evidence: During the inspection on July 31, 2017, there were not any reunification plans documented in the emergency preparedness plan. Plan of Correction: The site manager will work the Early Headstart Director and Education and Disabilities Coordinator to develop a reunification plan.
