Licensing Inspector arrived at 11:45 AM and departed at 2:00 PM to conduct an unannounced renewal in conjunction with a subsidy health and safety inspection. At the time of the inspection, there were 61 children (ages infant through school-age) in care with nine staff. Children were observed napping, reading books and involved in infant play. All center documents were also reviewed. The sample size was six staff and seven children's records. Violations were noted in the areas of administration, physical plant and programs.
Standard #: 22VAC40-185-160-A Description: Based on a review of six staff records, it was determined that the facility did not ensure that each staff member submitted documentation of a negative tuberculosis (Tb) screening no later that 21 days after employment. Evidence: Staff #1, date of hire 7/11/17, did not have documentation of a negative Tb screening on file. Plan of Correction: Staff will have Tb screening and results will be placed in the record.
Standard #: 22VAC40-185-270-A Description: Based on inspection, it was determined that the center did not ensure that all areas of the center, inside and outside, were maintained in a safe condition. Evidence: 1. There were strings on the carpet in the three-year-old room that were long and could pose a tripping hazard to children. 2. There were several hoses left unwound on the playground that could pose a tripping hazard to the children. 3. The posts that were supporting the canopy on the playground had areas of rust and peeling paint near the bottom. 4. The metal door from the playground to the three-year-old room was separating leaving exposed insulation and sharp edges exposed to the children. Plan of Correction: 1. The carpet strings were trimmed during the inspection. 2. The hoses were wound up during the inspection. 3 & 4. Work orders will be submitted to have work completed.
Standard #: 22VAC40-185-280-B Description: Based on inspection, it was determined that the facility did not ensure that all hazardous substances were stored in locked places. Evidence: There was a bottle of hand sanitizer sitting on a shelf in the four-year-old classroom. Plan of Correction: Hand sanitizer was locked during the inspection.
Standard #: 22VAC40-185-440-E Description: Based on inspection, it was determined that the facility did not ensure that there was at least 12 inches of space between occupied cots. Evidence: There were several cots in the two-year-old room that did not have 12 inches between them. Plan of Correction: Teachers will be retrained on proper cot placement.
