An unannounced inspection was conducted on August 7, 2017 to review subsidy health and safety inspection requirements. Nine staff were present with sixty-five children. Nine children's and six staff records were reviewed. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office. Standard #: VENDSUB-000-005-B
Description: Based on review, facility did not ensure for each group of children, there is a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: On August 7, 2017, 3 of 6 classroom's attendance sheets were not consistent with the number of children observed in the group.

Description: Based on review, facility did not ensure there is a written emergency preparedness plan that addresses staff responsibility and facility readiness with respect to emergency evacuation, relocation, lockdown and shelter-in-place procedures. The plan shall address the most likely to occur emergency scenario or scenarios, including but not limited to fire, severe storms, loss of utilities, natural disaster, chemical spills, intruder, and violence on or near the facility, and facility damage or other situations that may require evacuation, lockdown or shelter-in-place. Evidence: On August 7, 2017, the emergency preparedness plan did not address all required components for shelter-in-place and lockdown procedures.
