Comments:
Licensing Inspector arrived at 10:05am and departed at 2:15pm to conduct a Subsidy Health and Safety Inspection. At the time of the tour, there were 34 children, ages two years old through twelve years old. An additional six children arrived from a field trip during the inspection. Children were observed participating in classroom activities, playing outside, eating lunch, swimming, and during rest period. The sample size was five children and six staff.

Violations:
Standard #: VENDSUB-000-005-B
Description: Based on a review of documentation and interviews, it was determined that the vendor failed to maintain, for each group of children, a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: 1. The center did not have written documentation of the arrival and departure times for all of the children in care. 2. Administrative staff confirmed that there was not documentation of the arrival and departure times for all of the children in care.

Plan of Correction: The facility responded: Staff will record the time of arrival and departure for each child.

Standard #: VENDSUB-000-012-B
Description: Based on a review of seven staff records and interviews, it was determined that the vendor failed to ensure that staff employed prior to the effective date of this agreement shall complete the Virginia Preservice Training for Child Care Staff, to include all of the elements applicable to new staff, within 90 days of effective date of this agreement. Evidence: 1. Three of the seven records reviewed did not contain documentation the the Virginia Preservice Training for Child Care Staff was completed. Staff #2, #3,and #6 did not complete the required Virginia Preservice Training for Child Care Staff. 2. Administrative staff confirmed that three staff had not completed the Virginia Preservice Training for Child Care Staff

Plan of Correction: The facility responded: All staff missing the Preservice training will complete the training. All new staff will complete the Preservice training within 90 days of employment.

Standard #: VENDSUB-000-012-E-1
Description: Based on a record review, it was determined that the vendor failed to ensure that all staff obtain current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of the children. Evidence: 1. A review of seven staff records found that staff #1, staff #3, and #4 were lacking current certification in CPR. 2. Administrative staff verified that staff #1, staff #3, and #4 were lacking current certification in CPR.

Plan of Correction: The facility responded: All current staff will complete CPR certification. Staff who have completed certification, but were missing documentation will bring in their certification card.

Standard #: VENDSUB-000-012-E-2
Description: Based on a record review, it was determined that the vendor failed to ensure that all staff obtain current certification in first aid appropriate to the age of the children. Evidence: 1. A review of four staff records found that staff #1, staff #3, and #4 were lacking current certification in first aid. 2. Administrative staff verified that staff #1, staff #3, and #4 were lacking current certification in first aid.

Plan of Correction: The facility responded: All current staff will complete first aid certification. Staff who have completed certification, but were missing documentation will bring in their certification card.

Standard #: VENDSUB-000-031-B-2
Description: Based on a review of facility's emergency preparedness plan and interviews, it was determined that the vendor failed to ensure that the shelter-in-place procedures included procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child. Evidence: 1. The emergency preparedness plan for the facility did not include procedures for reuniting children with their parents or guardians within the procedures for shelter-in-place. 2. Administrative staff confirmed that the procedure for how they would reunite children and parents/guardians during a shelter-in-place was not in the emergency preparedness plan.

Plan of Correction: The facility responded: The procedures for shelter-in-place will be updated to include the procedures for reuniting children with their parents/guardians.

Standard #: VENDSUB-000-031-B-3
Description: Based on a review of the emergency preparedness plan and interview, it was determined that the vendor failed to ensure that the emergency preparedness plan includes procedures for lockdown. Evidence: 1. The emergency preparedness plan for the facility did not include all of the required lockdown procedures. 2. Administrative staff confirmed that all of the required lockdown procedures were not in the emergency preparedness plan for the facility.

Plan of Correction: The facility responded: The emergency plan will be updated to include of the procedures required for lockdown.
