Comments:
An annual subsidy inspection was conducted today. The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation. If any violations were cited, they were assigned a risk assessment and discussed with the provider/director. Standard #: VENDSUB-000-006-B-11
Description: Based on medication review, a written allergy care plan has not been maintained for each child with a diagnosed food allergy. Evidence: Child #1 and #2 had medications due to a diagnosed food allergy, neither children had a written allergy care plan.

Plan of Correction: We will obtain a written care plan for each child with a food allergy.
