Comments:
Today's visit was a "other" investigation due to a self-report incident regarding an allegation of a forbidden action. The incident was first reported to licensing on 8/1/17 and the investigation took place on 8/09/17. During the investigation video footage was reviewed, documentation regarding the incident was reviewed, photos were viewed, and interviews took place with the Program Director and one of the facility owners. The staff member directly involved in the incident was not available to be interviewed. Based on the evidence during the investigation, there was a violation of a licensing standard regarding a forbidden action and a standard was cited as a result of the investigation. Standard #: 22VAC40-185-410-1
Description: Based on the inspector's review of video footage and photos, the center failed to ensure a staff member did not engage in forbidden action according to this standard. Evidence: The inspector received a self-reported incident from the center regarding an allegation that a that a three year old child had been roughly handled by a staff member. The staff member is seen on video moving a child, while the child is seated in a chair, with one hand on the back of the chair and one hand around the child's top portion of the arm. The child was moved across the floor in this manner for approximately three feet. The child was in the chair for "thinking time" purposes and had moved the chair forward. The force used by the staff member to move the child while seated in the chair left marks on the child's arm. Attempting to move a child in this manner resulting in marks left on the child, is considered roughly handling a child.

Plan of Correction: Provide extensive training to all teachers on the proper way to handle children in lifting and when a child needs to move to another part of the classroom via video training, article training, and hands on demonstration.
