Comments:
An unannounced renewal inspection was done today August 21, 2017. The inspector arrived at 6:45 A.M. and departed at 8:30 AM. There were 35 children present today with four staff supervising. The inspector reviewed five children's and five staff's records during the inspection. Observed the children during: arrival, free play, gym time and breakfast. Discussion with the site director about: documenting the lead teacher training in the areas required, and ensuring that all first aid kit supplies are restocked.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, the center failed to ensure that all records were complete. Evidence: 1. The inspector found that child # 5 was missing the complete address for both required emergency contacts.

Plan of Correction: The site director will obtain the addresses ASAP and will monitor all records.

Standard #: 22VAC40-185-540-E
Description: Based on observation, the center failed to ensure that all emergency supplies were in working order. Evidence: 1. The batteries for the radio were no longer working.

Plan of Correction: The site director will obtain new batteries ASAP.
