Description: Based on observation, the center failed to ensure that staff and children's records were treated confidentially. Evidence: 1. The "Supplemental form" from the 2/10/17 inspection was posted with the other reports from that inspection. This form has information pertaining to staff and children's records. The form has a typed statement at the top "DO NOT POST OR NOT FOR RELEASE" and a standard form statement at the bottom that says that this form is not to be posted at the facility.

Plan of Correction: The site director removed this form during the inspection. The site director will ensure this form is not posted in the future.

Standard #: 22VAC40-185-60-A
Description: REPEAT VIOLATION Based on record review, the center failed to ensure that four of five children's records contained complete documentation of information record in the each child's record. Evidence: 1. The records of children #1,2,5 did not have documentation of a first date of attendance. 2. The records of children #1 and #5 who are siblings did not have documentation of a complete address for one of two required emergency contacts. 3. The record of child #2 did not have documentation of the mother or father's work phone numbers. Both had documentation of a work place.

Plan of Correction: The site director documented the first dates of attendance for all child records during the inspection. The site director will get the parents to document the information in each child's record. The site director will ensure child records are complete at enrollment.
