An unannounced renewal inspection was conducted beginning at 4:15 p.m. and ended at 5:45 p.m. with three staff. There were twenty-three children in care ranging in age from five years to twelve years of age. Children were observed returning from hand washing and preparing for snack. Snack today was half turkey and cheese sandwiches and water. After snack, children washed hands and played at their choice of centers that included imagination play, games, art and blocks. After centers, children went outside for physical activity. Seven children's records and four staff records were reviewed. Staff training was reviewed. Four medications were reviewed. Standard #: 22VAC40-185-260-B Description: Based on review of documentation, there was no documentation of an annual health inspection. Evidence: The most recent health inspection was dated 4/21/2016. Plan of Correction: The documentation could not be located during the inspection. This information will be requested from the school.
Standard #: 22VAC40-185-510-A Description: Based on review of documentation, the center failed to obtain written parental authorization for prescription medication. Evidence: There was no documentation of parental authorization for Child #5's prescription Penicillin. Plan of Correction: The documentation could not be located during the inspection, if it cannot be found, a new form will be requested from the parent.
