A Subsidy Health and Safety inspection was conducted on August 28, 2017 to review supplemental health and safety requirements. At the time of inspection, 51 children (12 mos through five years) were directly supervised by 7 staff and ratios were in compliance. The sample size consisted of 5 children records and 5 staff records. Children were observed during morning arrival, engaged in group sharing, and building with blocks. Handwashing procedures were also observed. There was an abundant supply of books, toys, and materials for the children. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-012-B Description: Based on record review, 4 of 5 staff members did not complete Virginia Pre-Service Training within the first 90 days of employment or subsidy vendor approval. Staff #2, 3, 4, and 5 did not have documentation of the completed Virginia Pre-Service Training. Plan of Correction: I will have my staff complete the online training.
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
