Technical Assistance:
Clarification was given regarding 22VAC40-185-(7)-500-B-3-e covered receptacle for soiled linens.

An unannounced monitoring inspection was conducted on August 30, 2017 from approximately 10:00 a.m. until 2:05 p.m. At the time of entrance there were 60 children and 12 staff members present with children. There were additional staff on site for kitchen and administrative purposes. The sample size consisted of 6 children's records and 8 staff records. Five medications were reviewed. Children were observed during center time, to include blocks, video games, puzzles and dress up. Schoolage children were also observed working on math and word review activities. Previous violations were reviewed. Areas of non-compliance are identified on the violation notice. The results of the inspection were reviewed and verified with the center program director during the exit interview. Please contact the licensing inspector with any question at 404-0568.

Standard #: 22VAC40-185-340-D
Description: Based on observation and staff interviews, the licensee did not ensure that in each grouping of children there was at least one staff member who met the qualifications of a program leader or program director . Evidence: During the inspection on August 30, 2017, in room #109 there was not a person with the 10 two year old children who met the qualifications for program leader from approximately 8:30 a.m. until 10:05 a.m. Staff #6 and #7 were observed working in the classroom. Staff #2 entered the room approximately 10:05 a.m. and meets the qualifications of program leader.

Plan of Correction: The program director will put a qualified staff member in the room today.

Standard #: 22VAC40-185-380-A
Description: Based on observation and staff interviews, the licensee did not ensure there was a posted daily schedule that allows for flexibility as children's needs require. Evidence: During the inspection on August 30, 2017, there was not a daily schedule posted for the schoolage children. Staff #1 confirmed the schoolage children do not leave the center for field trips and they stay at the center for daily activities .

Plan of Correction: The program director will have it posted today.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the licensee did not ensure the diaper changing surface was a nonabsorbent surface. Evidence: During the inspection on August 30, 2017, the diaper changing pads had rips in room 109 (3 small rips) and in room 107 (4 small rips) and the plastic covered pads with rips were no longer nonabsorbent.

Plan of Correction: The pads were replaced during inspection.

Standard #: 22VAC40-185-550-L
Description: Based on observation and staff interviews, the licensee did not ensure the center notified the parent by the end of the day of any known minor injuries with all required information. Evidence; During the inspection on August 30, 2017, 15 injury reports were reviewed and 7 did not contain the date the parent was notified of the child's injury and 6 did not include how the parent was contacted.

Plan of Correction: The program will update the forms today.
