Comments:
An unannounced monitoring inspection was conducted on this date on August 31, 2017. At the time of entrance there were 17 children and 8 staff members present. Additional staff were on site for administrative purposes and program support. The sample size consisted of 5 children's records and 5 staff records. There were not medications on site. The center's large playground is no longer being used as a part of the program. Children were observed eating breakfast and reading. The inspection began at approximately 8:05 a.m. and ended at 11:30 a.m. The area of noncompliance is noted on the violation notice. Standard #: 22VAC40-185-560-F-4
Description: Based on observation and staff interviews the licensee did not ensure children three years of age or younger may not be offered foods that are considered to be potential choking hazards. Evidence; During the inspection on August 31, 2017 three children under the age of 3 (two were age 2 and one was age 1) were obserserved eating whole grapes. The grapes were served as a part of fruit cocktail.

Plan of Correction: The assistant director will ensure that all teachers are informed of the violation and must cut grapes lengthwise.

Standard #: 22VAC40-191-60-C-2
Description: Based on observation and staff interviews, the licensee did not ensure employees were denied continued employment when a central registry finding is not obtained within 30 days of employment. Evidence: During the inspection on August 31, 2017, staff #4 was working during the inspection and documentation of a CPS check was not available for review. Staff #4 date of hire was 7-10-2017. The center's assistant director confirmed the staff was working and the documentation was not available for review.

Plan of Correction: The assistant director will ensure the background check will be placed in the record on September 5, 2017.
