Discussed with the licensee that children's records need to have parent's business address.

Licensing Inspector conducted a Subsidy Health and Safety Inspection from approximately 10:48am to 2:45pm. At the time of the tour there were 12 children, ages 8 weeks to three years old with four staff. Children were observed napping, coloring and playing freely. The sample size was six staff records and five children's records.

Description: Based on record review and interview, the vendor did not ensure for each group of children, the vendor maintained a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: There was not documentation of departure of each child in care.

Plan of Correction: The program director will reiterate to staff that the sign out is up to par.

Description: Based on and interview and review of procedures, the vendor did not have emergency preparedness plan with the all procedural components for evacuation and shelter in place. Evidence: During the subsidy inspection conducted on September 06, 2017, the vendor did not have the following procedures: Shelter in place procedures did not have accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during shelter in place and reuniting children with parents or authorized person designated by the parent to pick up. Lockdown procedures did not have accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during evacuation or relocation

Plan of Correction: The program director will updated in the next 30 days.
